Human Resources - 2008 Health and Dental Insurance Open Enrollment Information

The Office of State Comptroller - Retirement and Benefit Services Division has released new information relating to the 2008 Health and Dental Insurance Open Enrollment period.

This year the Employee Health Insurance Fair will not be held.  Each employee will receive a detailed Healthcare Options Planner for 2008-2009. All information will be mailed from the Comptroller's office to the employees' home address.

 

What you should know:   

This Open Enrollment period is an opportunity for employees to make changes to their health and/or dental insurance.

 

To Make a Change to Your Medical and/or Dental Plan:

Employees may make changes to their Medical Plan Provider, Medical Plan Level and/or their Dental Plan.  To make a change to your insurance, employees must complete a 2008 Benefits Open Enrollment Form.

 

The 2008 Open Enrollment Form Change form may be obtained by accessing the link below and submitting it to Human Resources or you may stop by the Human Resources Department to complete the form.

Due Date: **All changes must be submitted to the Human Resources Department no later

than 5 p.m. on June 6, 2007.  The Open Enrollment Change Form must be submitted  in

“good order”.**

 

Medical Provider Selection – (three providers available):

Anthem BlueCross Insurance

HealthNet Insurance

Oxford Insurance

 

Plan Level Selected – (three plan levels are offered):

Point of Service (POS) – allows in-network or out-of-network benefits

Point of Enrollment (POE) – in-network benefits only

Point of Enrollment-Gatekeeper (POE-G) – in-network benefits only/referrals required from primary care doctor.

 

Dental Provider Selection – (two providers and three plans available):

Cigna Dental Care® DHMO – in-network dentist only/employee must select dental provider/orthodontic benefits available.

UnitedHealthcare Basic Plan – no network/freedom to select dental office of choice.

United Healthcare PPO Plan – in-network and out-of-network benefits available/orthodontic benefits available.

 

Dependents:

During Open Enrollment, employees may also add eligible dependents not previously enrolled on the plan.    

 

Changes to This Year’s Insurance Benefits:

Caremark will be the State’s pharmacy benefit provider.  There are no changes to the State’s prescription drug benefits.

 

2008/2009 Premium changes:

The insurance rates for the period of July 1, 2008 to June 30, 2009 have been modified to reflect the premiums negotiated with the carriers.  For the new bi-weekly rates please click on the link below.
